MIDDLE FORK STABLE ACTIVITIES RELEASE AND WAIVER AGREEMENT In consideration of being allowed to participate in horseback riding at Middle Fork Stables, I, for myself and my heirs, personal representatives, or assigns, acknowledge the risks of injury or damage to property, personal injury, illness, paralysis, or death involved in horseback riding and any other scheduled or unscheduled activities. I understand that there is a risk in riding or being around live animals and acknowledge that my participation is purely voluntary. I assume full responsibility for any bodily injury, accident, illness, paralysis, death, loss of personal property, and related expenses resulting from any accident that may occur while I participate in horseback riding or any other activities at Middle Fork Stables. I agree to abide by all safety instructions. I hereby release, acquit, and forgive Middle Fork Stables, its family, heirs, employees, visitors, and volunteers from any and all liability of any nature for any injury or damage, including property damage, personal injury, illness, paralysis, or death, resulting from my participation. I acknowledge that these activities may involve a test of physical and mental limits and may carry the potential for death, serious injury, and property loss. I agree not to participate unless I am medically able. I also acknowledge that Middle Fork Stables operates independently of any other individual or entity involved in or owning any land, water, or air where activities are associated or planned. I expressly waive any claim, lawsuit, complaint, charge, or cause of action against the released parties for any injury or damage resulting from my participation. This waiver is made voluntarily. I have read this Release and Waiver Agreement and understand that by signing it, I am waiving valuable legal rights, including any rights I may have against the released parties.
